• Third-Party Billing Disclaimer (Headway): Root Cap Counseling utilizes Headway, a third-party billing service, to process insurance claims. Any benefit checks or cost estimates provided by Headway are soft inquiries and estimations only. Your final responsibility may change once your insurance carrier completes their standard 90-day audit. To avoid unexpected costs, we highly recommend contacting your insurance provider directly to verify your exact rates for services.

Couples Counseling (In-Person Only)

• Private Pay / Cash Rate: $210.00 per session

• Note on Couples Counseling: We offer couples counseling as a private cash-pay option only. Insurance is not accepted for couples counseling. Please note that all couples counseling sessions must be conducted in person; teletherapy is not available for this service.

Your Health Records Rights

Under Texas law and federal HIPAA regulations, you have the right to access and obtain copies of your clinical records.

• How to Request Records: To inspect or receive a copy of your health records, please submit a written request to Root Cap Counseling.

• Response Timeline: In accordance with Texas state law, we will respond to your written request within fifteen (15) business days of receipt.

• Fees: A reasonable, cost-based fee may be charged for the preparation, copying, or mailing of your records, as permitted by state regulations.

Consumer Complaints & Regulatory Information

If you believe Root Cap Counseling has violated state price transparency laws, mishandled your health records, or if you have a dispute regarding professional conduct or billing practices, you have the right to file a formal complaint with the state licensing board.

Root Cap Counseling is regulated by the Texas Behavioral Health Executive Council (BHEC).
